2024 Youth in Philanthropy Committee Registration

Please use the form below to register the participants of your school's Youth in Philanthropy committee.


As attendance at all YiP events is a mandatory part of the program, this information will be used as a basis for event RSVP.

Should circumstances change during the program and a participant cannot attend an event, please let us know.

This form captures image consent. Please share the Conditions of Use outlined below with parents/guardians of the participants and obtain their consent prior to completing this form.

In order to protect you and/or your child’s interest, as well as comply with the Privacy Act 1988, please read the following Conditions of Use:

CONDITIONS OF USE
Lord Mayor’s Charitable Foundation regularly collects, uses and reproduces images of people in its print and digital publications, including but not limited to the Connect and eConnect newsletters, promotional and marketing material, website and other Foundation communication channels (e.g. social media) in order to promote the Foundation’s granting programs and philanthropic services.

By marking the 'Image Consent' box below, you consent and provide continuing authority for the Foundation to take, collect, use, reproduce and publish images of you in its print and digital publications.

Photo Consent
I agree to Lord Mayor’s Charitable Foundation using and reproducing images of me in the manner explained above. I hereby release Lord Mayor’s Charitable Foundation from any and all claims, actions and liability relating to its use of said images.

Please provide the names, email addresses and image consent for students participating in this year's Youth in Philanthropy program. Each school committee can register up to ten participants.
